You are age 55 or older
You live within the Element Care PACE service area
You can live safely in the community with the help of PACE services at the time you enroll
You need enough assistance to qualify for a nursing home level of care, but want to continue living at home
Program costs depend on your income and assets. For individuals covered by Medicaid (MassHealth) only, or both Medicaid (MassHealth) and Medicare, there is no premium nor are there any co-payments.
For individuals with Medicare only, or with private insurance, there will be a monthly premium but there will be no co-pays or additional costs.

What does “all-inclusive” mean? It means a comprehensive, coordinated care plan personalized just for you by an interdisciplinary care team. Your team may include primary care providers, nurse practitioners, social workers, dietitians, physical, occupational and speech therapists and more.
We provide a wide range of services – from primary care and behavioral health to dental, vision and hearing. From safe transportation and meals to engaging activities at our adult day health centers, Element Care PACE focuses on improving your health and wellness every day.
Element Care PACE centers are located throughout the North Shore, Merrimack Valley, parts of Greater Boston and the northeast region of Middlesex County in Massachusetts.
