Element Care PACE is a nonprofit healthcare organization with a simple mission: to help aging adults live safely, comfortably and independently at home and in the community while limiting stays at hospitals and nursing facilities.
We get to know participants as individuals – who they are, where they live, what they like to do – so we can treat the whole person, not just the symptoms.
We work together as a team to discuss ideas and deliver compassionate, all-inclusive care that helps people live healthier, more fulfilling lives in their own communities. That’s what Element Care PACE is all about.
Element Care provides delegated care management, utilization management and transitions of care management to 1800 Commonwealth Care Alliance (CCA) Senior Care Options (SCO) members. CCA SCO is a health plan for older adults who qualify for both Medicare and MassHealth. SCO helps coordinate healthcare services and provides access to a network of doctors, specialists, hospitals, and support services while allowing members to keep their existing primary care provider. Members receive care management support, behavioral health services, prescription coverage, and additional benefits designed to help older adults remain healthy and independent in the community.

Founded in 1995, Element Care PACE was established as a joint venture of the Lynn Community Health Center and Greater Lynn Senior Services to bring the Program for All-Inclusive Care for the Elderly (PACE) to seniors in our community.
Today, we have seven PACE adult day health centers and two alternate care settings covering Greater Boston communities.

A Vision Begins
Element Care is founded with a mission to health older adults remain safely in their homes and communities through coordinated, compassionate care.
Opened Market Street site in Lynn, establishing the organizations first center and beginning its mission to serve older adults in the community.
2003 – 2008:
Early Growth & Expansion across the North Shore
2003: Opened Cummings center in Beverly.
2007: Opened Emerson center in Gloucester.
2008: Opened a new PACE center at administrative headquarters on Friend Street in Lynn.
Leadership: Bob Wakefield joined Element Care PACE as CEO.
2012
Building Community
Launched the first annual PACE Olympics, fostering engagement, fun, wellness, and community among participants.

Hosted Element Care’s 1st Gala celebrating 30 years of providing Healthcare from the Heart at the Seaport Hotel in October 2025.
