After joining PACE at Element Care in 2015, Diane worked closely with the Element Care rehab team and can walk long distances without any assistance.
To help with her depression and substance use disorder, Diane gets support from her care team to continue on the positive path. She also gets rides to a dermatologist three times a week and is now cancer free.

Element Care also provides her with companion services to help with grocery shopping. In an effort to quit smoking, she participated in Element Care’s Smoking Cessation group and made tremendous progress.

Diane’s improvements in her health have allowed her to spend more time with her grandchildren, socialize and make more friends at her PACE Center
