A Few Distinguishing Aspects of PACE at Element Care
PACE industry leader in integrating behavior health into primary care. Our behavioral health experts work closely with the primary care team in recommending therapies that benefit participant’s mental health as well as their physical health.
On-site social workers/case managers, physical, occupational and speech therapists, as well as dentist, podiatry & acupuncture to enhance service access.
Employing new technologies such as our avatar initiative to enhance companionship, medication adherence and reduce ER visits.


Provide skilled nursing services
Offer transportation to ADH and medical appointments to ensure socialization and care plan compliance
Deliver medications with easy to follow instructions to improve compliance and eliminate continual pharmacy pick-ups
Constant visual monitoring in day rooms by nursing staff to enable rapid clinical access and interventions
Broad range of activities including Tai Chi, Yoga, Cooking Classes, Olympics, and much more, to address the changing needs and interests of participants
Home health and home making services
Daily hot meals provided at our PACE Centers or delivered home
Cultural programs to engage diverse communities (Latino, Khmer, etc.)
