Provide Your Financial Information to Our Staff Member
You will need to include in the packet copies of any financial documents: a recent bank statement, copies of any retirement funds, copy of your monthly income, social security, SSI, SSDI, or pension, GIVE ALL THIS TO THE ENROLLMENT SPECIALIST.
MassHealth Processes Your Application
Staff will send all paperwork to Masshealth. Masshealth can take up to a month or more to process the application and at any time may request more information.
We’ll Contact You to Setup a Virtual Appointment
Team members that could call you: Medical Doctor, Nurse Practitioner, Behavior Health Therapist, Nutritionist, Physical Therapist, Occupational Therapist, Activity Specialist, Social Work for a virtual appointment.
Approval and Next Steps!
If approved, staff will drop off more paperwork for you to sign. Staff will coordinate a time to call you and discuss plan of care.
