A Team Approach
While some programs may offer you a care manager, Element Care believes that quality health care for our participants requires an entire team of clinicians and other professionals with experience in the needs and concerns of older adults. We treat you as a whole person, not a symptom or a condition. By working together, your team of highly skilled professionals can coordinate all your care and better support your overall wellness goals.
At Element Care, your team meets every morning to discuss any changes in your health and well-being and then works with you to develop a treatment plan and begins to implement it immediately.
If you have an accident or medical issue at night, you can call our 24-7 hotline and speak directly to an Element Care clinician who has access to your care plan and health history.
If necessary, we will send a nurse to your home or schedule a visit with your doctor, a physical or occupational therapist, or other professional at our Adult Day Health Center the next day and provide transportation for you. If you need to see a specialist, we schedule the appointment and transportation and ensure there is a seamless flow of information between the specialist and your primary care physician.
In addition to the care provided by your PACE team at the Adult Day Health Center, Element Care provides care in your home, access to a wide array of physician specialists, dental, hearing, and vision care, as well as emergency room treatment, inpatient hospital and end of life care.

Upon enrollment in PACE at Element Care, you and your care team create a personalized health care plan. It all starts with a comprehensive assessment with you and your family to gather information on your medical history, your cultural and spiritual values, and your interests and socialization needs. With that level of knowledge, we develop a care plan that addresses all of your needs and goals.
At the center of your care team is your doctor who is experienced with addressing the unique needs of older adults.
While the typical primary care physician has about 1,800 or more patients, your PACE doctor is only assigned about 150 patients. This gives them the time they need to get to know you like family and provide the type of highly personalized and responsive care to help you meet your health and wellness goals. And because of PACE’s low physician-to-patient ratio, you typically can see your PACE doctor or nurse practitioner the same day you get sick.
