THE ELEMENT CARE PACE PROMISE:
Improving Health and Enriching Lives
“We get to know our participants as individuals – who they are, where they live, what they like to do – so we can treat the whole person, not just the symptoms.”
“We deliver care as a team: physicians, nurses, occupational and physical therapists, social workers, behavioral health specialists, CNAs, transportation coordinators and more all work together, sharing information and ideas.
Our team thoroughly evaluates every eligible person on enrollment, then determines which services will best help the participant and their family. Delivering compassionate care that helps people live healthier, more fulfilling lives in their own communities. That’s what Element Care PACE is all about.”
