Tour Element Care: Meet the Team
To ensure that you find Element Care a good fit for your healthcare needs, we invite you and your family to tour the Adult Day Health Center closest to you. You will get to meet your primary care doctor and other medical professionals that may be part of your care team should you join the program. Transportation and lunch will be provided for the tour.
Approval
Our care team will discuss your medical needs and will assess if you can continue to live in the community safely. In addition, approval will be required by the state for your MassHealth application (if submitted) prior to enrollment in the program.
Welcome to PACE!
Once all appropriate approvals are obtained, an enrollment representative will meet with you to go over the enrollment agreement and get all required documents signed. Enrollment into the program will begin on the first day of the following month. For example, if you signed the agreement on June 20th, your official enrollment into the program will be July 1st.

Home Visit by Enrollment Specialist and Nurse
Once qualified by our Information & Referral specialist, a home visit will be scheduled. An Enrollment Specialist will come to your home to complete an intake interview and paperwork.
An Enrollment Nurse will also meet with you to learn more about your health history, current conditions and medications.
A Medicaid Specialist will meet with you to review your MassHealth status and will walk you through the required documents needed in order to submit your MassHealth application if needed.

Frequently Asked Questions
How to qualify for PACE?
To be eligible for PACE, an individual must be age 55 or older, want to live in the community but need enough assistance to qualify for nursing home care, be able to live safely in the community and live within our service area. Visit our eligibility page for more details.
If you do not qualify for Medicaid (MassHealth), can you still be eligible for PACE?
Yes. If a person meets the income and asset limits to qualify for Medicaid, Element Care pays for a portion of the monthly PACE premium; Medicare pays for the remainder. Individuals who do not qualify for Medicaid are responsible for the portion of the monthly premium Medicaid would pay. If you are unsure about your Medicaid eligibility, give us a call and our staff can help determine if you qualify.
Do you lose Medicare if you enroll in PACE?
Your Medicare coverage/eligibility is not affected by enrolling with PACE. You will continue to pay your Medicare premium, and Medicare in turn pays a portion to Element Care for the cost of PACE. If you wish to disenroll, your Medicare is quickly and seamlessly set back to how it was before you joined the program.
Does Element Care offer preventative healthcare?
Element Care’s mission is to keep our participants healthy and living in the community for as long as possible. In order to achieve this mission, the medical team at Element Care is very focused on providing preventative care. From teaching participants the value of nutrition to encouraging physical activity, our comprehensive care plan addresses all of our participants needs to keep them living at home safely and comfortably.
