PACE for Complex Patients
Element Care PACE is a coordinated care program for seniors 55 and up who qualify for nursing home care but want to continue living in the community.
Often, these patients require closer monitoring, more urgent appointments, or more frequent check-ins than the average practice can accommodate. Communication challenges, medication non-adherence and missed appointments may further complicate their care.
With PACE programs like Element Care, these patients receive comprehensive care from a multi-disciplinary team that meets daily. This approach may be a good fit for patients that require a level of care that isn’t compatible with an office-based practice.
