“Without all the doctors and the staff at PACE, I don’t know what I’d do. When I come here it is the only time I feel happy. Life is great today.”
– DIANE TITUS, PACE PARTICIPANT
Born in Lynn, and living in Peabody with her two cats, Khloe and Ophelia, Diane Titus has transformed her life.
Before joining PACE at Element Care, Diane struggled with substance use disorder for 20 years. She was frequently admitted to the hospital and had to undergo emergency surgery due to cirrhosis. She was also diagnosed with chronic kidney disease.
Her health continue to worsen and after her last admission in the hospital, Diane was told she had six months to live. She could barely walk and was in a wheelchair before joining PACE and was diagnosed with skin cancer.
After joining PACE at Element Care in 2015, Diane worked closely with the Element Care rehab team and can walk long distances without any assistance.
To help with her depression and substance use disorder, Diane gets support from her care team to continue on the positive path. She also gets rides to a dermatologist three times a week and is now cancer free.
Element Care also provides her with companion services to help with grocery shopping. In an effort to quit smoking, she participated in Element Care’s Smoking Cessation group and made tremendous progress.
Diane’s improvements in her health have allowed her to spend more time with her grandchildren, socialize and make more friends at her PACE Center
